Provides a broad range of service for the community that encompasses programs from early intervention for children to adult services including employment navigation. The program is available to individuals of all ages and all ranges of physical, behavioral, and developmental disabilities.
Community and Support Services include:
- Services for Children
- Early Intervention
- Assessments made in family’s or caregiver’s home
- Provide year-round services in child’s natural habitat
- Team includes: Developmental specialist, Speech and Language Pathologist, Occupational therapist, Physical therapist
- Autism Diagnosis Education Program (ADEP)
- PLAY Project (Play and Language for Autistic Youngsters)
- Home consultation model that provides monthly half day (3 hour) visits or bi-weekly 1.5 hour visits to train and support the caregiver in practical skills and structure
- Available for young children from birth to 6 years old
- Services for Adults
- Employment Navigation
- Case Management
- Service and Support Administrators
- Assists families through eligibility determination process
- Developing customized individual service plans (ISPs) for adults
- Assist with selection of service providers and monitor quality assurance
- Student to Adult Life Transition (ages 14 to 22 years old)
- Teaches individuals and families about a variety of topics related to growing up and post-high school activities – employment, training, and living
- Advocacy and Clubs
- Early Intervention
Costs:
Most of the programs provided are free, however, some fees may occur and require funding:
- Early intervention services are funded through Federal and State grants
- Local school districts are required by law to provide free and appropriate education
- Funding through Medicaid
Current operations during COVID-19
- Employees must take temperatures at home, if above 100 degrees, must stay home
- Employees working in the office must wear facial coverings and observe social distancing
- Disinfectant protocols will be implemented throughout the building every day
- No large gatherings or meetings will be held
- Home visits will still occur, however, families will be contacted within 24 hours to verify they are not experiencing any symptoms
- Employees will be provided with Personal Protective Equipment (PPE)
- Outdoor visits are encouraged when possible
- Skype, Facetime, Zoom will be considered
